I really don't think dean intended to lose the fight. there was certainly no indications that he was holding back.
I suppose on thing that strikes me is that of the fights you mention only in the Pilot is there really no active supernatural element involved. The other times we've seen this are (I think) when Dean has hit Sam in Bloodlust, and again in Metamorphosis - both times when Sam did not respond.
